Through a CDC MOU-partnered Center for Self-determination, Self-direction, and Self-care (CS-3) training unit, research, translation, workforce development and service delivery systems change, the Evidence-based Health Promotion Laboratory addresses integrated, person-driven evidence-based health and wellness offerings, delivered in-person and virtually, such as: diabetes prevention and management; chronic disease management, education and support programs; making the connection—a loneliness and social isolation focused intervention; falls prevention and physical activity programs such as Tai Chi for arthritis and balance; multidisciplinary dementia care and assessments, environmental redesign, and comfort and advanced care planning.

The lab also provides consultation to state units and city departments on aging and health and community partners on building communitywide intervention workforces, securing funding and regulation changes to better support needed services, and innovations in implementation of aspects of the Older Americans Act.
